Dear Sir,

My son, ____________ (Name) who was ____________ (Age) the time when he was issued Ration Card by your Office on____________(Date) has crossed the age of 12 on ____________ (Date)as per the Xerox copy of his birth certificate enclosed and accordingly he is now entitled to get an Adult Ration Card. I am sending herewith his Minor Ration Card with the request to arrange to issue Adult Ration Card for him in replacement, at your earliest convenience.
