Dear Sir,

This is for your kind information that I am the holder of Endowment Policy No ____________ I find that there is the omission of my name in the Notices and Premium Receipts which I have been receiving from your office excepting the address and Policy No. ____________ printed by computer,

This leads me to believe that proper care has not been taken to feed the input to the computer and hence the omission of my name. I would, therefore, request you to see that my name is inserted in the Notices and premium Receipts henceforward.
