This is to inform you that I have got my Savings Account No.____________ opened with your Bank in my short name as ____________ (Name) on the basis of the name mentioned in the enclosed Xerox copy of voter’s identity card. Whereas my full name is ____________ (Name) mentioned in my international passport.

Xerox copy of the relevant page of the same mentioning therein my full name is also enclosed. In this connection, I am sending herewith Affidavit for your kind perusal.

I would now request you to advise your concerned official to mention my full name as (Name) in your records deleting my short name and honour cheques/demand drafts when deposited which are received by me often in my full name or in the name by which I am operating my Savings Account No.____________ .
